Confirmation of Diagnosis

It is a syndrome of middle aged Caucasian males consisting of

  • Prolonged diarrhoea
  • Malabsorption
  • Arthritis
  • Weight loss

Arthritis: (90 % of cases):

Arthritis is mild, acute in onset, migratory and usually transient lasting for few days. Commonest joints involved are knees, ankles, finger, hips, shoulder, elbows and wrists.

Associated conditions:

Hypotention, lymphadenopathy, fever, edema, serositis (pleurisy, pericarditis, endocarditis), CNS involvement (loss of memory, confusion, depression, headache etc.)

Confirmation of diagnosis:

  • Histopathology of small intestinal specimen showing foamy macrophages containing PAS staining bacterial remnants of tropheryma whippelii or related bacteria PCR testing
  • Treatment:

    General symptomatic treatment for arthritis Trimethoprim sulfamethoxazole combination in long term therapy, cefixime therapy for CNS relapses.
